Liability Waiver

In consideration for participating in the classes made available by Good Mood Group (Classes), to the extent permitted by law, I release and forever discharge:

• GFD Health Pty Ltd ABN 84 619 291 324 (Good Mood Group); and
• any trainers or other wellness providers engaged by either Good Mood Group or The Zenith to provide the Classes, (together the Releasees), from any and all liability, claims, suits, demands, actions or proceeding arising out of or in connection with my participation in the Classes including, not by way of limitation, any claims arising from the negligence of any Releasees resulting in personal injury, accidents or illnesses (including death) and/or property loss and/or the use of any facilities or equipment provided in connection with the Classes.

As a condition of my enrolment, I accept full and complete responsibility for my own ability to healthfully participate in the Classes. I understand that participation in the Classes and the use of the facilities and equipment provided in connection with the Classes is potentially hazardous.
